Follow-Up Full Body Imbalance Scan

This is for clients who have already purchased a scan previously (either a full body imbalance scan or a hormonal/nutritional imbalance scan). 

After purchase, we will send you a convenient collection kit with supplies to return your hair and saliva.  You will receive a notification once your kit is ready for pickup or once we have shipped it out.  Once your samples have been collected, they must be returned to Nourish promptly.  If we have shipped out your kit, you are responsible for return postage to Nourish.

 You will receive a link after purchase to fill out your Nutritional Consulting Consent Form as well as your intake form.  Please sign this form and fill out your intake form.

Payment Plan Available!  Pay in two installments 4 weeks apart - use discount code SPLITPAYMENT.  First payment will secure your order and sample kit.  We will send out your sample kit and get your results to the lab.  We will then email you an invoice for the last half of the payment due 4 weeks after your initial payment.  Please note that payment needs to be made in FULL before your scan results will be released to you.
