You will receive a link after purchase to fill out your Nutritional Consulting Consent Form as well as your Intake Form.  Please sign this form and fill out your intake form.

Payment Plan Available!  Pay in two installments 4 weeks apart - use discount code SPLITPAYMENT.  First payment will secure your order and sample kit.  We will send out your sample kit and get your results to the lab.  We will then email you an invoice for the last half of the payment due 4 weeks after your initial payment.  Please note that payment needs to be made in FULL before your scan results will be released to you.
